Tone Decay Test is a specialized audiological assessment designed to evaluate the endurance of the auditory nerve when exposed to continuous sound. Unlike pure tone audiometry, which measures the softest sounds a person can hear, the tone decay test examines how long a patient can sustain the perception of a tone at a given intensity. This makes it particularly valuable in identifying subtle neural or retrocochlear disorders, such as lesions affecting the auditory nerve or brainstem pathways, which may not be detected through routine hearing tests. During the procedure, a continuous pure tone is presented at a level just above the patient’s threshold of hearing. The patient is asked to indicate whether they continue to hear the tone as time passes. If the tone “fades away” or becomes inaudible within a short period, even though it is still being presented, the audiologist increases the intensity and repeats the test. The degree of tone decay—how quickly the sound disappears from perception—provides important diagnostic information. A small amount of decay is considered normal, but significant or rapid decay may suggest pathology in the auditory nerve or central auditory system. Clinically, the tone decay test is most often used to differentiate between cochlear hearing loss (originating in the inner ear) and retrocochlear hearing loss (originating beyond the cochlea, often involving the auditory nerve). For example, patients with cochlear damage may show minimal tone decay, while those with retrocochlear involvement often demonstrate pronounced decay, even at higher intensities. This distinction is critical in guiding further medical investigations, such as imaging studies, and ensuring timely referral to neurologists or ENT specialists. The test is simple, non‑invasive, and can be performed quickly in a clinical setting. For patients, it requires only attentive listening and reporting, making it accessible across age groups. For clinicians, it provides an objective measure that complements other diagnostic tools like speech audiometry, impedance audiometry, and advanced electrophysiological tests. In summary, the tone decay test is a powerful diagnostic tool that goes beyond measuring hearing sensitivity to assess the stability of auditory perception over time. By detecting early signs of retrocochlear pathology, it plays a crucial role in ensuring accurate diagnosis, guiding appropriate medical management, and safeguarding long‑term auditory health.
